Starting Sunday, July 21, the Columbia Suicide Severity Rating Screen (C-SSRS) tool is being added to the required documentation for admission at all hospitals using Epic@UNC. The C-SSRS will replace the existing suicide screening.

Medication/Order reconciliation is required for Admission, Discharge, Transfer of level of care, and after procedures in the OR, Endo and Cath/EP labs when the patient is being moved to any inpatient nursing unit. If Med Rec is not completed, all ordered inpatient medications will go on MAR Hold, alerting nursing staff that Med Rec has not been completed.
