Effective Aug. 1: New and Revised General Consents, Privacy Practices and Acknowledgement, and Other Registration Forms

Beginning Aug. 1, 2015, new versions of the general consent, psychiatric general consent, notice of privacy practices, and patient rights will replace previous versions. The new versions should be implemented across the health care system, regardless of the use of Epic@UNC or not.

On Aug. 1, Epic@UNC will reflect these changes for any eSignature documents in the Document List. New pdf versions of the documents can be found at this Intranet site and were emailed to operational leaders for those who may be unable to access the Intranet site.

All hard copies of previous versions should be shredded and replaced with the current versions on or before Aug. 1. The Point of Service Forms document (Intranet link) details each form, and the highlights are listed below. Questions regarding the use of the forms or obtaining the .pdf’s if unable to access the Intranet site should first be directed to the operational manager/director and may be subsequently directed to Becky Neal at Rebecca.Neal@unchealth.unc.edu.

Highlights effective Aug. 1:

  • The HIPAA Notice of Privacy Practices (NPP) acknowledgement has been separated from the General Consent. The NPP requires acknowledgement via the new eSignature document once per version of the NPP; whereas, the General Consent is required at different intervals dependent upon the type of service (outpatient, inpatient, emergency department, etc.)
  • The General Consent has been revised and will now, for outpatient clinic and Wakebrook Crisis & Assessment visits, only prompt every 365 days. Witness signatures are no longer necessary and have been removed. The General Consent is still required for each visit for all inpatient, emergency department, and surgical services.
  • The Psychiatric General Consent has been revised and will now, for outpatient clinic visits, only prompt every 365 days. Witness signatures are no longer necessary and have been removed. The Psychiatric General Consent is still required for each visit for all inpatient, behavioral health emergency department, and Wakebrook residential visits.
  • Patient Rights must be offered and statused for every hospital and hospital-based clinic visit. Please be sure to use the entity-specific Patient Rights handout, as they differ among the entities. Patient Rights are not an Epic eSig document – only the status of the offer to the patient is necessary to record. The Patient Rights handout and status are not a requirement for UNCPN or other non-hospital-based clinics, however, we are unable to suppress the Epic warning message by department type. The warning may be ignored by UNCPN and non-hospital-based clinics.
  • The Spanish versions of the Important Message from Medicare and Tricare have been added as Epic eSignature documents.
