If you have a My UNC Chart account or activate one by July 21, you can take this short survey about your My UNC Chart experience and be entered into a drawing for a chance to win one of three FitBits. Full-time and part-time UNC Health Care and School of Medicine employees, faculty and physicians are eligible to win. As a UNC Health Care employee, you do not need to be a UNC Health Care patient to activate a My UNC Chart account, take the survey and be entered into the drawing.

If you don’t have a My UNC Chart account, it’s easy to get one. Go to myuncchart.org, click the “I Need An Activation Code” button and follow the steps. For step-by-step guide on how to activate your account, watch this short video.
