Careport Establishes Systemwide Patient Referrals and Patient Choice Management Process

In May UNC Health Care System went live with Care Port Guide and CarePort/AllScripts referral management platforms. CarePort guide, a patient facing tool allows care management to communicate in-person, via text or e-mail sharing information about skilled nursing facilities, home health agencies and other sites of care.

These tools provide care management staff improved methods to support patient, families and guardians in making decisions about care after the hospital based on quality, amenities and location. Careport/AllScripts referral management is part of a systemwide effort to establish a common patient referrals management process and to give patients and their families/caregivers enhanced capability for them to make informed choices for their continuing care services.

The implementation at the beginning of May was for Skilled Nursing Facilities, Long-term Acute Care Hospitals, Home Healthcare Services, and Rehabilitation Hospitals. The next phase will implement this tool for Assisted Living, DME and Infusion Services.

The initial implementation in May went live with the following capabilities:

  • Guide: a patient-facing tool to guide patients, families and guardians to make choices regarding continuing care based on  high-quality care
  • Referrals Management:  electronic process where care managers send referrals to continuing care sites and receive acceptances for patient care
  • Connect: first-time capability to manage patients across continuing care settings with real-time data and alerts based on patient location of care
  • Insight – first-time capability to evaluate patient outcomes along with continuing provider metrics for more effective relationship management

Project Sponsors were Linda Keenan (System Director, Care Management) and Sharon Kimball (Director, Continuing Care Network) and managed through Carolina Value as part of its Continuing Care Services portfolio of projects. Careport is expected to reduce our software costs by consolidating disparate patient referrals management software platforms while providing greater patient choice, improved care management processes and outcome measures.
