GRAChIE, DaVita Dialysis data now available in Epic@UNC via Care Everywhere

​Epic@UNC is now connected to the Georgia Regional Academic Community Health Information Exchange (GRAChIE)​ and the DaVita Health Information Exchange through the eHealthExchange, which means that this means that UNC Health Care providers can access clinical documentation from GRAChIE and DaVita patients in Epic@UNC directly in a patient’s chart.​

Coming in May: New Value Care Banner Colors in Epic@UNC Help Identify Exclusive/Narrow Network Populations

In early May 2019, new green-colored banners in Epic@UNC will identify patients aligned to an exclusive or narrow network Value plan, including the Cigna Connect ACA and BCBS Blue Value ACA plans. These two patient populations have limited or no out-of-network coverage, and their care should remain with a UNC Health Alliance provider or facility.

Senior Alliance ACO Medicare Beneficiaries identified by Value Banner in Epic@UNC

UNC Senior Alliance, the Accountable Care Organization (ACO) of UNC Health Care, began participation in the Next Gen ACO Model on Jan. 1, 2017. As an organization, we are responsible for managing the quality and total cost of care for the Medicare ACO beneficiaries attributed to UNC Senior Alliance providers. On Monday, Feb. 27, employees and providers will begin to see a value banner notification in Epic@UNC identifying Senior Alliance attributed ACO Medicare beneficiaries as either a ‘Value Care Patient’ or a ‘Value Care Patient – Complex.’

Longitudinal Plan of Care accessible for all providers/staff

All Epic@UNC users can now access the outpatient Longitudinal Plan of Care (LPOC) for each patient. The LPOC is automatically-generated based on data already in the patient's chart and includes a summary of the patient's care, goals, and past/future encounters, the patient’s care team, etc.

Patients’ able to sync data from iPhone ‘Health’ app to My UNC Chart, Epic@UNC

New ‘Track My Health’ features in My UNC Chart allow patients to share data from their iPhone’s Health app to My UNC Chart and their provider. Data for blood pressure readings, exercise logs and peak flow values can be entered into the Health app and flow into My UNC Chart if the patient’s provider has ordered the appropriate flowsheet.

In keeping with updated pneumococcal vaccine recommendations from the CDC, UNC Health Care's Population Health team and ISD are preparing updated adult Pneumococcal Vaccine Health Maintenance Topics in Epic@UNC for April 2016.
