The state of member health

N.C. HealthSmart programs and interventions are working! Members are better managing their chronic conditions. As a result, Plan savings totaled $18 million in 2007. This means that for every $1 spent on the Health Coaching program, $2 are saved.

Next, the State Health Plan will focus on wellness efforts to prevent chronic disease. These results and next steps are outlined in The State of Member Health: Trends and Interventions for Plan Members 2008.

In 2008, NC HealthSmart expanded program services to include the following:

 

  • Member outreach to address the five chronic diseases (diabetes, coronary artery disease, chronic obstructive pulmonary disease, asthma and heart failure) and additional “coachable” conditions including obesity, migraines, chronic pain and hypertension.
  • Intensive case management in now addressing high-risk pregnancies in addition to multiple hospitalizations and other catastrophic events.
  • The Plan is also looking at various formulas for disease prevention that specifically address smoking cessation and weight management.
