Acknowledgment and Reporting Form - Policy on Vendor Relationships in Biomedical Research or Patient Care
The Policy requires that all faculty, staff, and students of the School of Medicine and employees of the Health Care System acknowledge whether or not they have relationships with Vendors each year.

The Policy on Vendor Relationships in Biomedical Research or Patient Care, adopted jointly by the UNC Health Care Systems and the UNC School of Medicine, has been in effect since January 1, 2011. The Policy requires that all faculty, staff, and students of the School of Medicine and employees of the Health Care System acknowledge and report whether or not they have relationships with Vendors each year.
The annual acknowledgment and reporting form can be found at http://www.med.unc.edu/www/administration/policies, along with a copy of the Policy and Frequently Asked Questions. Any questions can be sent to the following e-mail address: vendorrelations@unch.unc.edu.
Related to this, the Compliance Office is working with the Medical Foundation of North Carolina to set up individual department accounts for Vendors to donate unrestricted funds. If your department has received donations from a Vendor and/or plans on receiving funds in the future, please e-mail your department name, department number and contact information to vendorrelations@unch.unc.edu. In addition, the Medical Foundation has set up a general fund that will be used for unrestricted donations from Vendors who do not designate a particular department. The Compliance Office will give instructions on applying for the funds once the account is set up at the Medical Foundation. Please e-mail any questions to vendorrelations@unch.unc.edu.
