In 2009, Duke Health Innovations, under Duke Clinical and Translational Scientific Awards (CTSA) auspices, funded 10 teams, each of which had a unique focus on combining Duke and Durham county expertise to improve the health of Durham residents.
The focus of the current presentation is on one of these teams – Healthy-in-Place (HIP) Seniors – and on one of its projects: developing a uniform minimum set of information, usable at all sites, that would complement medical inquiry and provide information helpful in maintaining personal independence in older community residents.
