Data includes Medicaid and Medicare claims as well as real-time data from 52 hospitals, lab results from lab service providers, and information collected from the primary care medical record, care managers, and patients.
Information is accessed by 14 regional Community Care networks and participating providers to identify patients in need of care coordination; to facilitate disease management, population management, and pharmacy management initiatives; to enable communication of key health information across settings of care; to monitor cost and utilization outcomes; and to monitor quality of care and provide performance feedback at the patient, practice, and network level.
This informatics framework is supporting primary care medical home and population management efforts statewide for over 1.5 million Medicaid, dual eligible, and HealthChoice recipients, as well as uninsured and commercially insured populations in several counties.
