UNC team receives American Society of Health-System Pharmacy Best Practices Award

A team from the UNC Internal Medicine Clinic and UNC Family Medicine Center were one of six teams to be selected for this national award.

Their project, titled "Advancing Pharmacy Practice through an Innovative Ambulatory Care Transitions Program," developed an interdisciplinary, outpatient clinic-based care transition intervention that reduced the hospital readmission rate from 20% to 6.5% for patients seen by a clinic-based pharmacist.
