Reminder: Requirement for SOM Faculty to Provide Transcripts by Jan. 31, 2017

As communicated by a memo from Dean Roper and Executive Dean Burks in the December 1, 2016 edition of Vital Signs, UNC Chapel Hill has enacted a policy that requires faculty members, including those in the School of Medicine, to provide official transcripts showing all graduate course credits for their highest degree.

As communicated by a memo from Dean Roper and Executive Dean Burks in the December 1, 2016 edition of Vital Signs, UNC Chapel Hill has enacted a policy that requires faculty members, including those in the School of Medicine, to provide official transcripts showing all graduate course credits for their highest degree.

As a reminder, each UNC School of Medicine faculty member must request that an Official Transcript from their highest earned degree be provided by January 31, 2017 to:

            UNC School of Medicine Human Resources

            Attn: Faculty Transcripts

            145 Medical Drive, CB# 9520

            Chapel Hill, NC 27599-9520        

Or, for institutions and transcript services which provide electronic official transcripts, faculty members may request that they email the electronic transcript instead to mike_mathews@med.unc.edu .

Information about the process to request a transcript is posted on the Academic Personnel Office website (http://academicpersonnel.unc.edu/ ).  Faculty will be reimbursed for the cost of obtaining an official transcript by their appointing academic department within the School of Medicine – please submit documents for this reimbursement to your department finance administrator.  Once all transcripts have been received, each department will then be reimbursed by the UNC School of Medicine Finance and Business Office.

Faculty members who, for reasons beyond their control, cannot provide an official transcript must complete the UNC Provost Office “Request For Transcript Waiver” form available online at: https://academicpersonnel.unc.edu/files/2016/12/Transcript-Waiver-Request-form.docx . The completed form must be approved and signed by the department chair before either mailing it to the UNC School of Medicine Human Resources’ address above or emailing it to mike_mathews@med.unc.edu .

We would also like to take this opportunity to thank all faculty members who either requested their transcript or provided a Request for Transcript Waiver form signed by their department chair.
