All clinical faculty members are asked to take this important survey. It will take only 15 minutes, and your feedback is essential.

While patient safety is difficult to measure, perception is considered a reliable indirect source of information. Measuring the perceptions of the organization and unit/area patient safety culture will help us to assess and manage risk.  In order for us to interpret the data in a meaningful way, we need a response rate of 60 percent or greater.  This year, the Patient Safety Culture Survey will have two versions: the Agency for Healthcare Research (AHRQ) Hospital survey for faculty and staff of UNC Hospitals, and the Medical Office version for hospital and community based clinics. Participation is essential for a complete and accurate understanding of the patient safety culture.
