With the implementation of Epic@UNC, there will be a new process using Epic@UNC to submit requests to the UNC Medication Assistance Program (MAP). Effective April 4, 2014, Medication Assistance Request Forms (MARF) will no longer be accepted.
With the implementation of Epic@UNC, there will be a new process using Epic@UNC to submit requests to the UNC Medication Assistance Program (MAP). Effective April 4, 2014, Medication Assistance Request Forms (MARF) will no longer be accepted.
All medication assistance requests can be forwarded to the UNC MAP team via the referral functionality within Epic. Epic referrals will be used for:
- Outpatient Clinic Infusion Prior Authorizations
- Benefits Investigations for:
- Outpatient Clinic Infusions
- Specialty Medications filled at a UNC Owned Pharmacy
- Copay Assistance for Specialty Medications filled at a UNC Owned Pharmacy
- Manufacturer Assistance for uninsured patients receiving:
- Outpatient Clinic Infusions
- Specialty Medications

There are 3 options to create and submit referrals to the UNC MAP Team:
- Submit an Oncology Outpatient Infusion Referral
- Submit a Non-Oncology Outpatient Infusion Referral
- Submit an Outpatient Specialty Medication Referral
