All Epic@UNC users can now access the outpatient Longitudinal Plan of Care (LPOC) for each patient. The LPOC is automatically-generated based on data already in the patient’s chart and includes a summary of the patient’s care, goals, and past/future encounters, the patient’s care team, etc.

- A key piece of the LPOC is the Care Coordination Note (CCN), which is a shared note that persists across ambulatory encounters and can provide information such as a summary of care, concerns, and next steps.
- The LPOC does not replace each inpatient’s Plan of Care, but is a valuable information source for Care Managers, inpatient/ED clinicians, specialists, and others.
- Components of the LPOC are also available to patients in both their outpatient After Visit Summary (AVS) and via My UNC Chart.
