Mahaney named new Director of UNC Home Health and REX Home Services

I am very pleased to announce Melissa Mahaney, RN, BSN MSN as the new Director of UNC Home Health and REX Home Services effective August 14, 2017.

Melissa is a highly-skilled, masters-level Registered Nurse with over 10 years of leadership experience in the home health industry. She has extensive experience integrating home health services within acute and post-acute care settings through partnerships with Accountable Care Organizations (ACOs), Clinically Integrated Networks (CINs) and emergency departments. Melissa has had great success leading multi-state transitional change initiatives including implementing, standardizing and integrating EPIC Electronic Medical Records (EMRs) for two health systems, 1100 providers and over 5000 patients served.

Prior to joining UNC Health Care, Melissa worked as the Area Director of Professional Services at Almost Family, Inc., one of the largest home health organizations in the United States, where she provided oversight of process improvement, quality improvement, and clinical operations across multiple home health agencies. Additionally, Melissa served as the Enterprise System Director of Clinical Performance and Management at Bon Secours Health System managing the clinical quality of home health agencies in a five-state region.

In her new role, Melissa is responsible for leading, improving and expanding all UNC Home Health and REX Home Services operations, programs and services. Key aspects of her role include planning for and addressing systemic and programmatic challenges, identifying and leveraging resources through interdepartmental collaborations, developing and implementing standard operating procedures, and guiding staff to reflect exemplary professionalism, service excellence and compassion for all patients served.


Faucher named new Director of UNC Hospice

I am very pleased to announce Sara Faucher BSN, RN, MSN, GERI-CNS as the new Hospice Director for UNC effective July 31, 2017.

Sara is a highly skilled executive who brings extensive experience in hospice/homecare management, operations, staff development, risk management and quality improvement. Sara, a Certified Hospice and Palliative Nurse (CHPN) and independent palliative care consultant, has had great success starting-up, redesigning and expanding hospice and palliative care services for multiple freestanding and hospital-based health care organizations. She also has extensive experience collaborating with community outreach programs, implementing marketing strategies, and building strong sales and clinical teams.

Prior to joining UNC Health Care, Sara worked as the Interim Hospice Executive Director for Simione Healthcare Consultants where she led efforts to integrate hospice services across multiple systems of care including Accountable Care Organizations (ACOs), hospital-based and hospital-affiliated organizations, nurses associations and small proprietary agencies. Additionally, Sara has served as Executive Director of Mercy Hospice and Palliative Care, one of the top five large U.S. health systems in 2017 (Truven), where she successfully doubled their census through the creation of new innovative programs and establishment of palliative care consulting services.

In her new role as Director of UNC Hospice, Sara is responsible for directing all hospice operations, programs and services. Key aspects of her role include budget management, nursing team oversight, service expansion and performance enhancement as well as leading change initiatives to improve the provision and quality of patient care.


Knott named new Regional Director for UNC Medical Center and UNC REX

I am very pleased to announce Barbara J. Knott, RN, MPA, ACHCE as the new Regional Director for Post Acute Services for UNC Medical Center and UNC REX effective July 31, 2017.

Barbara is an accomplished healthcare executive and a true leader in the Post-Acute field. She has extensive experience developing and maintaining relationships with sub- and post-acute care subject matter experts, medical and nursing administrators, and executive boards. Barbara is well versed in the delivery of both hospital-based and freestanding post-acute services and has had tremendous success in guiding quality and cost improvement efforts for multi-state, multi-facility health systems including Palliative Care, Hospice and Home Health agencies

Prior to joining UNC Health Care, Barbara worked at Kaiser Permanente as the Regional Executive Director of Home Care Services and has served as the Corporate Vice President for Bon Secours Health System; a $3.3B nonprofit health system.

In her new role as Regional Director, Barbara is responsible for the strategic and operational planning, direction, management, and coordination of UNC Post-Acute services. Key aspects of her role include assisting with the development and implementation of Value Based Reimbursement initiatives such as Bundled Payment modalities, Population Health Management efforts such as Accountable Care Organization (ACO) activities while continuously optimizing business practices to better support patients served by UNC Health Care.
