The General Consent for Treatment forms have been revised and will be effective on Friday, Sept. 21. After that time, all patients will need to sign the revised forms on their next visit.

- Epic@UNC e-signature forms have been updated as well, and Epic@UNC will prompt users to have patients sign the new version
- Please discard all hard copies of prior versions and replace with the newly revised versions:
