Participate in 5-minute Survey on Side Effects of COVID-19 Vaccination for a Chance to Win a $50 Amazon Gift Card.
Researchers in the UNC School of Medicine are soliciting participants for study on the side-effects of COVID-19 vaccination and the impact of those side-effects on work related absences. We would appreciate your participation in a brief, 5-minute survey about your experience with vaccination.
If you were offered the vaccination and didn’t opt to get it, you are also encouraged to participate and to tell us why you opted out of vaccination.
All data is collected anonymously and your participation is optional. However, those who choose to participate will be eligible to participate in a raffle for one of four $50 Amazon gift cards.
