Effective March 1, the North Carolina Department of Health and Human Services (NC DHHS) will require all death certificates to be submitted electronically using its new online system, the North Carolina Database Application for Vital Events (NCDAVE).

This system requires a unique username and password separate from an Epic@UNC login.

UNC Health currently is working with a vendor to register and create an NCDAVE account for ​each credentialed UNC Health physician and APP at all entities except for UNC Health Blue Ridge and CarolinaEast Health System.

Physicians and APPs at included locations should not sign up for their own NCDAVE account or they will have two accounts.

Once an account has been created, UNC Health physicians and APPs will receive an email from donotreply@vitalchek.com with a username and temporary password. Do not delete the vitalchek email without reading it. After reading the email, use the username and temporary password to create a new password and then save both for use starting March 1, 2021.
