UNC Health’s current, three-year agreement with Cigna expires on Nov. 30, 2025.


CHAPEL HILL, N.C.July 1, 2025 – UNC Health is seeking a new and fair agreement with Cigna and is warning employers and patients that their coverage may change later this year.

UNC Health’s current, three-year agreement with Cigna expires on Nov. 30, 2025. If UNC Health doesn’t reach a new and fair agreement, all Cigna commercial plans will be considered out of network beginning Dec. 1, 2025. That could mean higher costs for Cigna members who visit UNC Health providers, clinics and hospitals.

Leaders at UNC Health have engaged with Cigna regarding the expectations for a new agreement for nearly two years, with the goal of completing these discussions well in advance of the Nov. 30 expiration date. Unfortunately, Cigna is not willing to engage in meaningful discussion or commit to patient care that UNC Health requires. UNC Health is urging employers who have chosen Cigna and Cigna members to be aware of this situation as it unfolds over the coming months.

While UNC Health hopes to come to a mutually beneficial agreement with Cigna prior to expiration of the current agreement, during upcoming enrollment periods, employers and patients with Cigna coverage may want to consider alternative plans that include UNC Health in network for all of 2026.

For now, nothing changes. Cigna members should continue to visit UNC Health hospitals, facilities, and physicians as usual. They can continue to schedule appointments and receive care at all UNC Health facilities and from all associated UNC Health healthcare providers.
