Viewing 'Patient-Center Medical Home' video no longer required during Primary Care Provider premium credit activity for State Health Plan enrollment

Because many members have not been able to view the Patient-Center Medical Home video, watching the video is no longer a required part of the Primary Care Provider (PCP) premium credit activity during the 2016 Annual Enrollment in the State Health Plan. Members will not be penalized for not viewing the video.

The State Health Plan is working on this issue and encourages members to continue to try and view the video when they enroll, but members WILL NOT be penalized for not viewing the video. You will receive this premium credit as long as you have selected a Primary Care Provider (PCP) for yourself and each participating dependent.

Be sure to complete your health assessment and complete the tobacco attestation as well to secure the lowest premium (80/20 & CDHP plans), and receive the benefits of Health Insurance Special Pay (80/20 plan only). The 70/30 plan does not require premium credit activities.
