Revisions to ambulatory AVS 'Additional Plan of Care Education' for patients with 'Abnormal' BMI

Beginning Friday, Aug. 5, ambulatory clinic patients with a calculated ‘Abnormal’ BMI (based on recommended clinical guidelines) will see BMI definitions and recommended lifestyle changes included within the ‘Additional Plan of Care Education’ section of their After Visit Summary (AVS).

NOTE: The language below has already been utilized for patients ≥ 40; beginning 8/5/16, this documentation will apply to all patients ≥ 18 (‘Abnormal’ BMI ranges now specific to ages 18 – 64, and ≥ 65).


Specifications are based on patient age and current BMI calculations:

Underweight BMI—Both conditions are required to trigger this section of the AVS:

  • (A) Patient Age=18 to 64 AND (B) BMI= < 18.5 kg kg/m2
    OR 
  • (A) Patient Age ≥ 65 AND (B) BMI< 23 kg kg/m2

Overweight BMI—Both conditions are required to trigger this section of the AVS:

  • (A) Patient Age=18 to 64 AND (B) BMI= ≥ 25 kg/m2
    OR 
  • (A) Patient Age ≥ 65 AND (B) BMI ≥ 30 kg/m2

In addition to supporting enhanced patient education, these plans of care will aid in meeting new clinical guidelines for Physician Quality Reporting (PQRS) and other measures, and will help ensure UNC Health Care continues to score well among other organizations reporting quality metrics.
