The UNC Palliative Care at Home program is temporarily closing for a full reorganization on Friday, June 28. As a result, no new patient referrals to Palliative Care at Home will be accepted and appointments for currently enrolled patients will not be scheduled past Friday, June 28.

All Palliative Care at Home program patients have been given this list of alternative palliative care services available:

  1. UNC Outpatient Clinic – 984-974-4462 or 800-862-4938
  2. Hospice of Alamance-Caswell (Palliative division) – 336-532-0100 or 800-588-8879
  3. Liberty Homecare and Hospice Services – 800-999-9883
  4. Transitions LifeCare – 919-828-0890 or 888-900-3959

On May 21, we began reaching out to each patient individually to assist with their transition to the most appropriate service on the list above. In addition, all patients have been mailed a letter informing them that they will be offered assistance with transfer to their new program of choice. Patients who did not respond were contacted again on June 7. Patients without primary care or with more complex levels of needs will be transitioned to a new primary care provider and resources that will support their situation.

All patients will be transitioned to other programs not later than Friday, June 28. Our goal is for the new Palliative Care at Home program to relaunch in the fall of 2019.
