To decrease the potential for medication errors and also increase efficiency in the Pharmacy, effective Wednesday, May 4, the Epic@UNC team will implement dose ranges and standardized doses for multiple drugs administered to pediatric patients.


To decrease the potential for medication errors and also increase efficiency in the Pharmacy, effective Wednesday, May 4, the Epic@UNC team will implement dose ranges and standardized doses for multiple drugs administered to pediatric patients.

The drugs included in this dose standardization protocol are:

  • Acetaminophen oral products,
  • Most oral beta lactase,
  • Most IV beta lactase,
  • Diphenhydramine,
  • Decussate,
  • Famotidine,
  • Ibuprofen suspension,
  • IVIG,
  • Infliximab,
  • Polyethylene glycol, and
  • Sulfamethoxazole/trimethoprim.

Any calculated dose falling within the ranges identified in this file (Intranet link) will automatically change to the standardized dose. Page 3 also includes the steps needed for providers/pharmacists to override the standardization when clinically appropriate.
